Practice Transformation
The Practice Transformation (PT) program strengthens the HIV clinical workforce and improves patient outcomes across the Southeast. Working with select clinics, we implement patient-centered care models that integrate HIV and behavioral health services. PT supports quality improvement through goal-setting, workforce development, culture change, and tailored systems redesign.
